Cheshire Medical Center uses the myD-H patient portal used by the Dartmouth-Hitchcock Heath System.

What does this mean for Cheshire Medical Center Patients?

  • It means one convenient portal where you can view your health information related to visits at Cheshire Medical Center in Keene, Cheshire’s satellite offices in the Monadnock Region, and visits at other Dartmouth-Hitchcock locations. (Easy-to-access information is simply better).
  •  In addition to scheduling appointments, appointment requests, prescription refills, and messages to providers—the myD-H portal allows you to manage billing statements, electronically message questions to billing, and make payments
  • If you had our previous portal, FMH/myD-H, and do not have a prior Dartmouth Hitchcock myD-H portal from another DH location, you will need to set up a new myD-H account. FMH is no longer connected to Cheshire Medical Center's electronic record.
  • Information from 11/4/2017 to present will be available in the appropriately labeled options in our current myD-H portal (Visit notes, test results, medications, appointments etc).
  • Most of your Cheshire Medical Center health information from 1/2013-11/4/2017 will be available in the myD-H portal in an option called “Historical Documents”.
  • If you manage health care for a minor child or serve as a health care proxy for a family member you can request proxy access to their record via our current myD-H portal. You must first have your own account. On the home page of the portal there is a notice asking, “Are you a parent or guardian Or want access to another adult’s record?”.  You select the appropriate response and follow the prompts.
