Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) is pleased to announce that José Montero, MD, MHCDS has joined the Community Health Department as the Vice President of Population Health and Health System Integration.  Prior to joining CMC/DHK he served as the Public Health Director for the New Hampshire Department of Health and Human Services, Division of Public Health Services where he was responsible for strategy, policy setting, program development and financial planning. He has also served as State Epidemiologist, and Chief of the Communicable Disease control section.

Dr. Montero is developing and directing Population Health strategies by integrating clinical initiatives with community programs supporting the Monadnock region in becoming the healthiest community in the nation. In addition, Dr. Montero will serve as the principal liaison for CMC/DHK to Dartmouth-Hitchcock for all matters relating to Population Health. He will provide leadership in extending the CMC/DHK Healthy Monadnock 2020 model and related initiatives into other communities served by the Dartmouth-Hitchcock system.

Dr. Montero holds a Doctor of Medicine from National University of Colombia in Bogotá, Colombia and a Master of Healthcare Delivery Services from Dartmouth College in Hanover, New Hampshire. He also studied Family Medicine at Valle State University in Cali, Colombia. He is currently an adjunct professor at the Geisel Medical School at Dartmouth College and the University of New Hampshire, School of Health and Human Services. He also serves as Chair of the Centers for Disease Control and Prevention (CDC) think tank for Social Determinants of Health; and is a member of the Institute of Medicine Roundtable on Population Health Improvement the CDC’s Office of Infectious Diseases Board of Scientific Counselors; and the Advisory Committee to the Director of CDC.
