Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) is pleased to welcome Carole Putnam, APRN, to our Department of Family Medicine.  Prior to joining CMC/DHK, Putnam worked at Valley Family Physicians in Claremont, New Hampshire working with patients with acute illness and performing well child checks and routine adult physical exams.  Carole has over thirty years combined experience as a registered nurse and nurse practitioner which has broadened her professional expertise and knowledge in caring for patients throughout their lifespan.

Carole completed the Family Nurse Practitioner Program at the University of Massachusetts in Boston, Massachusetts. She also holds a Post Graduate Certificate in Nursing Education from Indiana University in Indianapolis, Indiana, a Master of Science in Parent-Child Nursing from Marquette University in Milwaukee, Wisconsin, and a Bachelor of Science in Nursing from Alverno College in Milwaukee, Wisconsin.  Ms. Putnam’s primary focus has been on children and their families and she has worked in a variety of settings including providing nursing education, working as a school nurse and as a staff nurse.  

At CMC/DHK, nurse practitioners work with primary care physicians and other members of the medical staff providing care in a team environment known as a medical home. A medical home empowers patients to be active participants in their health and well-being. The medical home team integrates all aspects of a person's preventive, acute, and chronic care, offering patients comfort, convenience, and optimal health throughout their lifetimes. CMC/DHK implemented a medical home model as a component of the healthiest community initiative, to help prevent and manage chronic conditions. It is one way CMC/DHK is working to make Cheshire County the healthiest community in the nation by 2020.
