Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) is pleased to welcome Kathryn Scoville, APRN, to our Department of Family Medicine. Prior to joining CMC/DHK, Scoville worked as a Nurse Practitioner at Covenant Family Practice and Urgent Care in Clarksville, Tennessee, and Blanchfield Army Community Hospital at Fort Campbell, Kentucky.  Kathryn has over twenty years combined experience as a registered nurse and nurse practitioner.

Kathryn completed the Family Nurse Practitioner Program at Vanderbilt University in Nashville, Tennessee, with a Master of Science in Nursing degree.  She also holds a Bachelor of Science in Nursing from Colby-Sawyer, in New London, NH, and a Master of Arts in Human Resources Development from Webster University, in Saint Louis, Missouri.  Ms. Scoville has worked in a variety of specialties including oncology, women’s health, internal medicine, urgent care, and chronic care management. 

At CMC/DHK, nurse practitioners work with primary care physicians and other members of the medical staff providing care in a team environment known as a medical home. A medical home empowers patients to be active participants in their health and well-being. The medical home team integrates all aspects of a person's preventive, acute, and chronic care, offering patients comfort, convenience, and optimal health throughout their lifetimes. CMC/DHK implemented a medical home model as a component of the healthiest community initiative, to help prevent and manage chronic conditions. It is one way CMC/DHK is working to make Cheshire County the healthiest community in the nation by 2020.
