Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) is pleased to welcome Diane Sette, APRN to our Department of Family Medicine.  Prior to joining CMC/DHK, Sette worked as a Clinical Practice Coordinator at the Ruttenberg Cancer Treatment Center at Mount Sinai Hospital and Medical Center.  She has worked as a nurse in a variety of specialties including oncology, internal medicine and women’s health.  Her interests include preventive medicine, women’s health, obesity management and promoting a healthy lifestyle.

Diane received her Master of Science, Family Nurse Practitioner Degree from The College of New Rochelle, and her Bachelor of Science from the College of Nursing at Northeastern University, Boston, MA.

At CMC/DHK, Ms. Sette works with other medical practitioners providing care in a Medical Home Team. The Medical Home Team integrates all aspects of a person's preventive, acute, and chronic care, offering patients comfort, convenience, and optimal health throughout their lifetimes. CMC/DHK implemented the Medical Home model to help prevent and manage chronic conditions.
