Champion Awards Nomination
Looking to highlight a Champion for their hard work and successes in the past year?
Our Champions will be recognized with the following awards:
H.E.A.L (Healthy Eating Active Living) Award: An Individual, Organizational, or School Champion honored for exemplifying healthy eating and active living behaviors within their family, neighborhood, work place, town, school or other group.
Healthiest Community Champion of the Year Award: An Individual, Organizational, or School Champion honored for inspiring healthy behavior change in others or advancing the action plan of the Healthiest Community initiative.
