Where does my health information come from?

Your information is obtained from your CMC/ DHK electronic medical record. Please note, if you already have a myD-H account for Lebanon or another Dartmouth Hitchcock location, you still
need a myD-H Keene account as these are different systems.

How do I register for the Patient Portal?

Enrolling in the Patient Portal is quick and easy. Here are the steps:

  1. Fill out an Application Form and show your ID at one of our office locations.
  2. In 7-10 business days, an email invitation will be sent to you. If you have not received your invitation in that time frame, check your spam folder. This invitation will expire after 90 days.
  3. Once you receive your invitation, click on the link in the email to sign up. The sign-up page has step-by-step instructions. You will create a unique username and password. Please don’t share your username and password with anyone.
  4. You will receive a second email confirming your connection with CMC/DHK.
  5. Welcome to myD-H Keene Patient Portal! You are now able to check your test results and access your electronic medical records at your convenience.
What do I need to access the Patient Portal?
  • Access to a computer and the internet
  • An email address
  • An internet browser capable of supporting 128 bit US encryption, such as Internet Explorer, Google Chrome, Firefox, or Safari
Is my medical information safe on a Patient Portal?

The myD-H Keene Patient Portal uses the latest security measures to ensure that your medical information remains confidential between you, the hospital and your care team. You will set your own password to secure access to your patient portal. Maintaining your privacy is our top priority and we have taken the necessary steps to ensure we meet your expectations as well as all federal and state privacy laws. In compliance with federal patient privacy regulations, all transmitted data is encrypted during this process. The data storage is also encrypted for protection. Your login occurs over Secure Socket Layer (SSL, shown as HTTPS in your browser’s address bar), which is an industry standard providing secure communication over the internet used by sites such as online marketplaces or financial institutions.

Can I pay my bills online and view my invoices?

You can pay your clinic bill,(not hospital) online with the myD-H Keene portal. The invoices are not viewable online, you must have your invoice with you so that you can enter your guarantor or record number and the amount you are going to pay.

Is FollowMyHealth™ secure?

Usernames and passwords are stored separately from clinical data within the cloud. The first time that you create a FollowMyHealth™ record using one of the Open Authentication accounts (Facebook, Yahoo, Google, or Microsoft Account), you will be asked to share some basic information from your connecting account so that FollowMyHealth™ will be allowed to use them for authorization. Rest assured that the only information obtained from this request is your username. FollowMyHealth™ will NOT be utilizing the potential public information contained in your authenticating accounts pictures, contacts, friends, etc. and will NOT release and/or sell your identifiable information to other organizations, people, etc. To be clear, the authentication provider with which you choose to use as your login, will NOT have access to your FollowMyHealth™ record or its information.

How do I change my password?

You can change your password by using any of these options:

I know my password:

  • Login to myD-H Keene.
  • Go to “My Account” at the top of the page and choose “Preferences” from the drop down menu.
  • Choose “Login Preferences” and then “FMH Secure Login Preferences” and follow the instructions.

I don’t know my password:

  • Go to the myD-H Keene patient portal login page and click on “Forgot your password?” The instructions will guide you through the process OR
  • Contact support services at 888-670-9775 and request a new password.
How do I view my health information?
  • Login to the myD-H Keene Patient Portal. Go to the CMC/DHK home page and click on “Login to Patient Portal” under the myD-H Keene logo to get to the Log In page. Then choose your login/ authorization choice.
  • To access electronic medical information, click on the “My Health” tab at the top of the screen. This tab will open a new set of tabs with specific health information.
  • You can click on “Results” to get more detailed information on your lab and x-ray results.
  • The “Documents” section contains office notes, after visit summaries, discharge instructions and patient education from your hospital visit. You must have a PDF reader to view or download a document.
  • The “Medications” section contains a list of your medications as listed in your clinic medical record.
  • Note that only information created after January 1, 2013 is accessible via the portal. To obtain copies of earlier records, contact the Medical Records Dept. at (603)354-5453.
What if my medical information is incorrect?

If you feel there is incorrect information in your medical record, you can call the Health Information Management Dept. at 603-354-5453 between 8 AM and 4 PM, M - F.

What if I want to view my children’s information?

Full proxy accounts will be granted to parents of children up to age 13 with all information available. For teens 13-17, privacy laws require limited proxy access. Once a child turns 18, the proxy will be discontinued and the 18 year old can apply for their own myD-H Keene account.

What if I need more information on a medication or condition?

In your record under “Medications” and “Conditions”, there may be more information available by clicking the "blue i" symbol. If you have additional questions, please contact your healthcare provider.

Why do I see the portal referred to as myD-H Keene and FMH?

myD-H is a Dartmouth Hitchcock system wide name for the online patient portal. CMC/DHK uses the portal product FollowMyHealth™ to deliver our internal Electronic Medical Records to your myD-H Keene account.

What if I need assistance with the portal?

If you have questions or concerns about how to find information on the patient portal, please call Follow My Health at 1-888-670-9775. They are available for you 24 hours a day, 7 days a week.
