More than just a name change, myD-H is a whole new way for you to manage your health care and become an even more effective member of your health care team. Patient Online(POL) is used by nearly 100,000 Dartmouth-Hitchcock patients to take charge of their health care. Everything you can do in Patient Online you will be able to do in myD-H, and it's still free to use. myD-H is used by nearly 100,000 Dartmouth-Hitchcock patients to take charge of their health care.
On the myD-H home page, choose Request an Access Code on the left hand side to complete two short forms, selecting your Dartmouth-Hitchcock location and entering in your registration information. Your request will be sent to our Medical Records department to confirm you are a registered patient and a new account will be created for you upon this approval. Check your mailbox within 2-5 business days and you will find a letter with your login information and one-time password. Simply use this information to login and designate your own, new password to ensure confidentiality. You are then able to begin using myD-H.
Please note that myD-H communications are limited to non-emergency interactions with Dartmouth-Hitchcock Keene physicians, physician services and billing.
