Mission

The CHaD Family Advisory Council of Keene works in partnership with CHaD leadership to promote and enhance family-centered pediatric care and services.

The Council will work...

  • To educate the community about Medical Home and Family Centered Care. Click here to learn more about the Medical Home concept.
  • To bridge the gap between families and their medical home to improve the health of children by providing input and influence with regard to policies, programs, practices and affecting the delivery of care and services in a family-centered environment.
  • To heighten awareness of specific disabilities and promote effective medical home care for children with special medical needs.
  • To encourage networking within the community and surrounding communities
  • To become stronger advocates for our children and families.
  • To promote quality communication between families, medical care team, and their community
  • To educate and empower families to be active partners in their children’s care

The American Academy of Pediatrics (AAP) describes the medical home as a model of delivering primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective.

Each month, the Council discusses ongoing initiatives and assigns follow-up directives. Members are required to attend the monthly meetings and to work on issues outside of Council meetings.

Amy Rounds
Amy is a proud parent of two children. Her daughter was diagnosed at birth with Trisomy 21 (Down syndrome). She was subsequently diagnosed with three heart defects that needed repair when she was ten weeks old. She is an active member in the Down syndrome community and is a support person through the Parent to Parent network in Vermont. She is an advocate for all children with special needs with a focus on Down syndrome. She is always willing to meet with new or expectant parents to discuss the many rewards and the common challenges of this diagnosis. For more...

Annie DiSilva
Annie is the proud mother of three boys (16 years old, 15 years old and 12 years old). Although Annie has an Associate’s Degree in Early Childhood Education, a Bachelor’s Degree in Elementary Education, and recently completed the Autism Spectrum Disorders Certificate Program at Antioch University, she views her personal experience of the Autism Spectrum, her most valuable education. Gail Dubriske
I live in Keene, have 4 children ages, 22, 27, 29, & 30 and 4 grandchildren ages, 5, 3,18mos & 10mos. Two of my grandchildren see Dr Rubin. I have worked as a receptionist in Pediatrics for 7 years and have loved every minute. I joined the Family Advisory Council last year. I am interested in hearing from the parents about what we can do to improve a family’s visit in Pediatrics. I have been on our Patient Satisfaction Committee for 2 yrs. This committee broke down all the patient satisfaction surveys and reviewed each department by department and question for question. I think being part of the Family Advisory Council and hearing the voice of our patients will help me make more positive changes.

I am a mother of three and have been a Pediatrician since 1995. I have wanted to be a Pediatrician since I was a kid. I have my own special health care need being born without one hand. Kristen Lippincott
Ever since I can remember I wanted to become a nurse. I started out my career in medical surgical nursing and eventually became an operating room nurse. Although I have loved all of my experiences in nursing I was always searching for an opportunity to have more of a voice in order to make a bigger impact on patients and families lives. Currently I am the Clinical Leader for Pediatrics and love that I have a voice that allows me to make positive changes for my team and our practice. I became I member of the Family Advisory Council so that I could listen to the suggestions of parents and try to implement them in the Pediatric department to make it a better place for all families.

Lisa Luciani
As a dedicated and loving mom, Lisa appreciates the special care and personal relationship her children have with the Pediatric Team at Dartmouth-Hitchcock Keene. Lisa brings to the Council nearly 20 years of professional marketing experience, as well as website design and administration. Lisa also brings a special relationship with Rise…for baby and family, a Monadnock United Way agency. Lisa currently serves on the Board of Directors at Rise…for baby and family. Rise offers early support services for infants and toddlers to 3 years of age. Michelle Miner
I joined the Family Advisory Council when I came on board as a Pediatric & Adolescent Care Coordinator in April 2009. As a Care Coordinator I get to work very closely with patients, families, and providers. Being involved with the FAC allows me the opportunity to hear what is important to our patients and be involved in making positive changes within our Medical Home.

Misty Martinez-Bohannon
Misty, mom to Abby born April 2002, who has multiple chronic disorders that include: Intractable Epilepsy Seizure Disorder, SCADD-Short-Chain Acyl-CoA Dehydrogenase Deficiency (a mitochondrial disorder), Developmental Delay, Asthma, Gastric Reflux- which resulted in a Nissen Fundoplication, Feeding Disorder- Swallowing problems which resulted in a g-tube placement at the age of 2, Delayed Gastric Emptying, Hypothyroidism (short stature), & Eyelid Ptosis w/ Astigmatism w/ hyperopia which resulted in glasses.
