Behavioral Health Specialist, Heather Shlosser, APRN, Joins Family Medicine Department at CMC/DHK

Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) is pleased to welcome Heather Shlosser, APRN, to our Family Medicine Department as a Behavioral Health specialist. Heather holds a clinical doctorate in nursing practice from the University of Virginia in Charlottesville, VA, a Post-Masters of Science in Nursing with a specialty in Psychiatric Mental Health from the University of Virginia, a Master of Science in Nursing from Frontier Nursing University in Hyden, KY, and a Bachelor of Science in Nursing from Elms College in Chicopee, MA. She has completed certifications as a Family Nurse Practitioner-Board Certified (FNP-BC) and Psychiatric-Mental Health Nurse Practitioner-Board Certified (PMHNP-BC) from the American Nursing Credentialing Center (ANCC) and is in the process of completing a Functional Medicine certification from the Institute of Functional Medicine. Professional memberships include NH Nurse Practitioner Association, American College of Nurse Practitioners, American Academy of Nurse Practitioners, National Organization of Nurse Practitioner Faculties, and the Institute of Functional Medicine.

Heather will provide behavioral health care as part of the team within Family Medicine assisting patients in developing self-management skills.  She will collaborate with primary care physicians and other members of the medical staff providing care in a team environment known as a Medical Home and putting together a plan that is related to a patient’s specific medical conditions and individual goals and lifestyle. The CMC/DHK Medical Home empowers patients to actively participate in their health and well-being and takes a proactive approach to working with patients to achieve the best possible health outcomes.
