Island Street Family Medicine Transition

Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) will move its Island Street Family Medicine practice to its main campus at 580-590 Court Street on July 1. The move will consolidate all Keene-based Family Medicine care into one location at the CMC/DHK main facility. CMC/DHK’s strategy for the project is to keep individual provider and nurse teams together where possible to ensure a seamless transition for patients.

“We are excited to welcome our Island Street patients, staff, physicians and associate providers to our Court Street location,” shared Andy Tremblay, MD, Department Chair, Family Medicine. “Island Street patients can now enjoy the many advantages of receiving care on-site at CMC/DHK’s main campus, such as radiology services, expedited lab services when needed, same day mammography and physical therapy services. We understand the change this represents for our patients, and the staff and providers that have worked at Island Street, some for many years. For our patients, we look forward to continuing the excellent care that they have received. For our staff and providers, we look forward to the opportunity for more daily interaction and the resultant professional growth that comes from these interactions.”

Island street physicians and associate providers will join the existing Medical Home care teams located at the clinic’s main office. Staff began notifying patients of the move in early June.

In December 2012, CMC/DHK announced that it would be working to reduce its operating budget by $7 million over an 18 month period. “This consolidation project is part of CMC/DHK’s commitment to becoming a sustainable health system and providing high quality, cost-effective care,” shared Don Caruso, MD, MPH, Medical Director of Dartmouth-Hitchcock Keene. “When the private family medicine practice on Island Street joined the Dartmouth-Hitchcock Keene clinic in 2004, we did not have space for the practice at our Court Street location. In recent years, a redesign and renovation of the Clinic space on Court Street took place and now we have room, allowing us to not renew the Island Street lease,” shared Caruso. “As we work to become a sustainable health system, Cheshire Medical Center/Dartmouth-Hitchcock Keene is focusing on keeping our patients well and promoting long-term health. In order to continue to meet our mission and maintain financial stability as an organization, we performed a thoughtful and extensive analysis and have decided to consolidate facilities to realize savings and increased efficiencies. We realize this will be a change for our patients, providers and staff and appreciate their understanding and patience during this transition.”
