Kate Ingram, APRN Joins CMC/DHK Department of Hospital Medicine

Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) welcomes Kate Ingram, APRN, Family Nurse Practitioner, to the Department of Hospital Medicine. While completing her Master of Science in Nursing and her certification as an Adult and Gerontological Nurse Practitioner, Ms. Ingram worked in the Wellness and Hospice Departments of Home Healthcare, Hospice and Community Services in Keene, NH. She has also worked with Oncology patients at Concord Hospital in Concord, NH, and at Cedarcrest Center for Children with Disabilities in Keene.
Ms. Ingram earned Certification as an Adult and Gerontological Nurse Practitioner from the American Nurses Credentialling Center and completed her Master of Science in Nursing at the University of Massachusetts in Lowell, MA. She holds a Bachelor of Science in Nursing from the University of Massachusetts in Boston, MA, and an Associate Degree in Nursing from New Hampshire Community Technical College in Claremont, NH. She is a member of the Oncology Nursing Society, Sigma Theta Tau: The International Honor Society of Nurses.
As an Adult and Gerontological Nurse Practitioner, Ms. Ingram may diagnose and develop a plan of care for patients with common as well as complex medical conditions, working closely with physicians. She has experience caring for patients aged 16 and above, providing health-maintenance guidance and care including annual physicals as well as specialty care for patients with cancer and patients with terminal illness. She works with primary care physicians and other members of the medical staff providing care in a team environment known as a Medical Home. The Medical Home Team encourages patients to take an active role in their well-being and integrates all aspects of a person's preventive, acute, and chronic care, offering patients comfort, convenience, and optimal health throughout their lifetimes. CMC/DHK implemented the Medical Home model to help patients prevent and manage chronic conditions.
