Dartmouth-Hitchcock Health

Founded in 1893, Dartmouth-Hitchcock Health (D-HH) is a nonprofit academic health system serving segments of the population of nearly two million individuals residing in New Hampshire and Vermont. (D-HH consists of Dartmouth-Hitchcock, affiliates, and their subsidiaries.) D-HH delivers care in Lebanon, New Hampshire at Dartmouth-Hitchcock Medical Center; the Norris Cotton Cancer Center; the Children’s Hospital at Dartmouth-Hitchcock; the 24 Dartmouth-Hitchcock clinics that provide ambulatory services located across New Hampshire and Vermont, and through Visiting Nurse and Hospice for Vermont and New Hampshire.  In addition, affiliate hospitals of D-HH include Alice Peck Day Memorial Hospital in Lebanon, Cheshire Medical Center in Keene, and New London Hospital in New London, New Hampshire, and Mt. Ascutney Hospital and Health Center in Windsor, Vermont.

eD-H

eD-H is the name for the Electronic Health Record (EHR) built on Epic software which is used by the Dartmouth-Hitchcock Health system.  Much like our patients, every hospital and health system is unique.  Epic components are developed with input from medical staff and employees to provide functionality best suited to the work and patient care provided in each specific area.  The customized EHR system of the Dartmouth-Hitchcock Health system and Cheshire Medical Center is referred to as eD-H.

EHR/Electronic Health Record

An electronic health record (EHR) is a digital version of a patient’s paper chart. EHRs are real-time, patient-centered records that make information available instantly and securely to authorized users.

Epic

Epic Systems Corporation, or Epic, is a privately held healthcare software company.  The Epic electronic health record (EHR) system is used by some of the nation’s largest and most prestigious hospitals and health systems.

myD-H

myD-H patient portal is a secure online website that gives you convenient 24-hour access to personal health information from anywhere with an Internet connection. Using a secure username and password, you can view health information.

myD-H Keene

The myD-H Keene patient portal was established to provide patient portal benefits to patients at Cheshire Medical Center in Keene.

  • Discharge summaries
  • Medications
  • Immunizations

  • Lab results
